Closer to Home

Closer to Home is a project bringing support for families affected by dementia closer to their local communities.

Thanks to our partner, Leeds Building Society, we’re bringing dementia care closer to home.

Families living with dementia can access practical, emotional and clinical support from a dementia specialist Admiral Nurse, at a dedicated time and place that suits their needs.

If you’re caring for someone living with dementia you can book an appointment with an Admiral Nurse, in confidence. Admiral Nurses have the skills, knowledge and experience to talk about, amongst others:

  • understanding the diagnosis, plus practical tips and advice for caring for a loved one with dementia
  • transition into nursing or residential home, or stays in hospital
  • understanding symptoms and changes in behaviour, and managing your own feelings

We will initially be providing virtual clinics and are working towards holding these face to face.

This is a brand-new project for Dementia UK and we’re delighted to be delivering this project with the help of Leeds Building Society. Bringing additional support to local communities through our Admiral Nurses.
