    The Chapel House Dementia Resource Community operates in the local community, helping people with dementia to stay at home for longer. It is also available in other settings, including care homes. Admiral Nurses work in partnership with family carers and people with dementia.

  • Earl Mountbatten Hospice Service

    To access this service the person being cared for must have a diagnosis of advanced dementia and be registered with a GP on the Isle of Wight. The patient should have or is anticipated to have complex and/ or end of life needs that can't be met by their current caring team.

    Referrals may be made by: the Isle of Wight NHS Admiral Nursing Team (part of the memory service); Primary Health Care Team members and other SPC teams; Hospital Consultants, Hospital Nurses or Clinical Nurse Specialists, e.g. cancer specific Nurse Specialists; or any health or social care professional involved in the patient’s care. Referrals can be accepted by post, telephone or email.
