  • Sid Valley (hosted by Royal Devon & Exeter NHS Foundation Trust)

    The person with dementia must be registered to the Sid Valley Practice (Sidmouth) and have a diagnosis of dementia (or a probable advancing dementia which was not diagnosed in the early stages). The Admiral Nursing service works intensively with carers, where the carer is providing a significant level of care.

    The service supports families affected by dementia with complex care needs, from admission through to discharge. The person with dementia must be an inpatient or outpatient at North Devon District Hospital.

    To access this service, the person with dementia must be registered with one of the GP practices in Purbeck (Bere Regis, Corfe Castle, Sandford, Swanage, Wool, Wareham). The carer must be providing a significant level of care and consent to the referral.

    Patients should have active, progressive and potentially life-threatening illness, including dementia.
    Patients should have unresolved, complex needs that cannot be met by the current caring team, or it is anticipated that the patient will develop such needs in the near future. These needs may be psychological, social, spiritual or physical

    Primary Health Care Team members, other SPC teams, Hospital Consultants, hospital nurses or Clinical Nurse Specialists, e.g. cancer site specific Nurse Specialists, any health or social care professional involved in the patient's care, the patient, a relative or friend.

    Referrals may be made by: the Isle of Wight NHS Admiral Nursing Team (part of the memory service); Primary Health Care Team members and other SPC teams; Hospital Consultants, Hospital Nurses or Clinical Nurse Specialists, e.g. cancer specific Nurse Specialists; or any health or social care professional involved in the patient’s care.   • The Admiral Nurse at Weston General Hospital will support families affected by dementia with complex care needs across the hospital setting, from admission through to discharge.

    The Admiral Nurse will support families where:
    • The person with dementia has a formal diagnosis
    • The person with dementia is an in-patient or out-patient at Weston General Hospital
    • The referrer has gained consent from the carer and/or the person with dementia (or has undertaken a best interests decision) prior to making the referral
    • The person with dementia has complex care needs and/or the family’s needs are complex; for example, due to high levels of distress / change in presentation of the person with dementia, or managing loss, transitions and changing relationships

    Referrals can be made by healthcare professionals across the hospital and carers will also be able to access the services of the Admiral Nurse via a regular Carers’ Café.
