    The Chapel House Dementia Resource Community operates in the local community, helping people with dementia to stay at home for longer. It is also available in other settings, including care homes. Admiral Nurses work in partnership with family carers and people with dementia.
    The family carer must be registered with a GP in and/or residents of the following locations within Cheshire West CH64, CH1 (Shotwick, Woodbank, Capenhurst & Mollington), CH66 (Ledsham) and South Wirral CH63 Bebington & Clatterbridge, CH61 Irby, CH60 Heswall. A service will be offered for people with a diagnosis of dementia.

  • The Admiral Nurse at Weston General Hospital will support families affected by dementia with complex care needs across the hospital setting, from admission through to discharge.

    The Admiral Nurse will support families where:
    • The person with dementia has a formal diagnosis
    • The person with dementia is an in-patient or out-patient at Weston General Hospital
    • The referrer has gained consent from the carer and/or the person with dementia (or has undertaken a best interests decision) prior to making the referral
    • The person with dementia has complex care needs and/or the family’s needs are complex; for example, due to high levels of distress / change in presentation of the person with dementia, or managing loss, transitions and changing relationships

    Referrals can be made by healthcare professionals across the hospital and carers will also be able to access the services of the Admiral Nurse via a regular Carers’ Café.

    This community-based service covers Honiton and the surrounding area and the person with dementia must be registered with a Honiton GP.     A referral to the Admiral Nurse service can be made via your GP, community Mental Health Team or if there is a community matron involved. All referrals will be triaged for suitability and inappropriate referrals may be referred to the Dementia Support Worker Service or Devon Carers.

    This service supports carers/ families of people living with dementia who are being assessed by the Northamptonshire Adult Social Service- Holistic Intermediate Care Team (HICT). To access this service the person living with dementia must be registered with a GP within the county of Northamptonshire and must be accepted by HICT. The Admiral Nurse Service supports families for up to 8 weeks.

    Referrals are sent directly to the HICT team following a hospital discharge.   • Isabel Hospice Admiral Nurse Service

    This Admiral Nurse Service covers East Hertfordshire and supports people with advanced stages of a dementia and their carers/families, where the person with dementia has reached the last year of their life and/or is experiencing complex symptoms. The Admiral Nurse is able to offer advice about how to manage symptoms, provide practical support and sign post to other services where appropriate. They are also be able to support people who are dying and co-ordinate care so the person can die in their preferred place of care and support their loved ones. The Admiral Nurse will also teach and educate others both within the hospice and externally by working closely with other hospices and providers to deliver these.
