The assessment uses the Decision Support Tool (DST), which evaluates needs across 12 domains, such as behaviour, nutrition, and cognition. Ratings from low to priority are assigned in each area, influencing eligibility for CHC funding.
Domains of Care Considered in CHC
Each of the 12 domains requires detailed information about the person’s needs. Key examples include:
