This is a community based service where residents of the North Tyneside area can make referrals for support with matters relating to dementia. The service will conduct a triage on assessment and will refer on appropriately to an Admiral Nurse, a dementia Coordinator or supply advice and information. Referral Criteria: •The person cared for has a diagnosis or suspected diagnosis of dementia •The carer or person with dementia must live in North Tyneside •The carer has agreed to referral being made. Reasons for referral includes: •Where family dynamics adversely affect care of the person with dementia •Significant difficulty with adjusting to diagnosis •Where there are significant changes to mental and/or physical health of the person with dementia •Carer stress, anxiety and/or depression •Carer physical health problems •Managing risk •Practical care skills training for the carer, information and advice •Professional advice and skills training to statutory and other agencies relating to supporting a specific person with dementia and their carer •To offer psychological support/advice to carers of people with dementia during periods of transition in the dementia pathway for eg making future care decisions/end of life care decisions •When the carer needs help to express choices and needs •To act as an advocate for the carer in liaison with other organisations and services. Exclusion criteria •Referrals specific to the person with dementia only and not the carer •Head injury that is primary diagnosis to cognitive impairment •Crisis management •Carer's primary need is not related to dementia.
